The path after an IVF failure

Going through infertility, then an IVF, followed by an IVF failure can be a very disturbing and shattering phase for childless couples eager to have a child. Preimplantation Genetic Screening/Diagnosis (PGS/PGD):

Genetic screening of embryos before transfer may be advised in cases of;

  • Women with chromosomal abnormality or women above 35 years of age.
  • Men with sperm DNA damage, abnormal sperm morphology.
  • Women with IVF miscarriages.
  • Risk of any genetic abnormality that can be passed to their child like Kilenfelter’s syndrome, cystic fibrosis, Y chromosome deletions, etc.
  • Unknown causes of recurrent IVF failure (RIF)

PGS/PGD can improve pregnancy rates and reduce chromosomal defects.

Use of frozen embryos/eggs: Couples with an IVF failure can use frozen embryos or eggs in their consecutive IVF cycles. It is observed that the clinical pregnancy rate is higher for frozen embryos than the fresh embryo transfer.

Blastocyst transfer: Blastocyst is a more developed embryo that has developed to day 5 or more where the cellular components and the fluid cavity are clearly distinguishable. Transfer of blastocysts into the mother’s uterus enables a more efficient selection of embryos and thus can enhance pregnancy chances.

Intracytoplasmic Sperm Injection (ICSI): Intracytoplasmic Sperm Injection (ICSI) A procedure of ART that can be done in cases of male fertility issues and fertilization failure reasons. In this procedure, the sperm is directly injected into the egg’s cytoplasm.

Assisted hatching is a fertility technique, used as part of IVF treatment, to facilitate the embryo hatching and implantation process.It may be helpful in women who are older or who have had a prior IVF failure.

Embryo glue: It’s a specially developed formula that contains, high levels of hyaluronic acid.It makes secretions stickier, aiding the process of implantation.Using Embryo glue has been shown to benefit couples who are aged over 35 years, those who have poor quality embryos and who have had three unsuccessful embryo transfers

Donor Programme: Sometimes, fertility treatment goes futile where one cannot use their own gametes for IVF due to various reasons. In such cases, the use of donor eggs or sperm can be an option. For couples with compromised egg and sperm quality, embryo donation can also be considered.

Surrogacy: Gestational carrier is another option of IVF procedure where the intended couples can take the help of a woman (surrogate) who chooses to carry their child for the pregnancy term. There are legal procedures along with the medical protocols that need to be abided by both the intended parent and the surrogate.
